A North Franklin Township woman already facing charges for allegedly stealing more than $200,000 from her former employer was arrested by Chartiers Township police Wednesday on more charges for allegedly forging her boss’ signature on paperwork she submitted to the state Department of Revenue.

Carrie Ann Ammons-Daviduk, 39, of 1285 Overlook Drive, was arraigned before District Judge David Mark on charges of attempted theft by deception, willfully filing false and fraudulent tax documents, two counts of tampering with public records and forgery. She was taken into custody at her home by township police.

Ammons-Daviduk was charged in December 2014 by Chartiers police with more than 100 counts of theft, theft by deception, tampering with records, unlawful use of a computer, forgery and failure to make required disposition of funds for allegedly stealing $209,471 from Valley Bulk, 919 Western Ave., while working as controller of payroll for the company. She had been scheduled to enter a plea but instead has opted for a trial.

Township police Sgt. Michael Filosi said he learned from both John Washabaugh, owner of Valley Bulk Transport, and the state Department of Revenue that Ammons-Daviduk reportedly claimed to be an independent contractor as part of the original theft scheme. As an independent contractor, she would have received a 1099 form, and would have been subject to self-employment tax.

Filosi said she did not pay taxes on the money she allegedly took from Valley Bulk and, when contacted by the state, sent a letter on Valley Bulk letterhead. She allegedly signed Washabaugh’s name to the letter indicating he had taken care of paying the tax.

Ammons-Daviduk was placed in Washington County jail on $100,000 bond. She is scheduled for an April 13 hearing before Mark.
